If you have already activated your AT&T Wireless phone with Mid-Atlantic ProTel, please take a moment to complete the registration form below so that we will be able to process your AT&T Promotion Card.

If you have not yet activated your AT&T Wireless phone, please visit a Mid-Atlantic ProTel store near you. The locations are listed in the link bar to the left. We look forward to assisting you in activating your phone and registering you with the High 5 Referral program.

It will take up to 30 days to confirm your new AT&T wireless number in our billing system. You and your friend will be automatically notified by email when you have been successful in earning an AT&T Promotion Card.
